IMPORTANT NOTICE:

PLEASE NOTE THAT THE LAST SURGERY OPENING DAY BEFORE CHRISTMAS IS FRIDAY 22nd DECEMBER.

TO ENSURE YOU RECEIVE YOUR PRESCRIPTION FOR REPEAT MEDICATION BEFORE THE CHRISTMAS BREAK, PLEASE ENSURE YOU SUBMIT IT BY WEDNESDAY 20th DECEMBER

WE CANNOT GUARANTEE THAT REQUESTS SUBMITTED AFTER THIS TIME WILL BE PROCESSED BEFORE THE SURGERY CLOSES ON FRIDAY 22nd DECEMBER.

THANK YOU

Repeat Prescriptions

Nice_vlue_pillsIf the Doctor has decided that you may have some of your regular medication without seeing him/her each time, the right hand side of your prescription will have a list of those items.

To obtain a new prescription, bring or post this slip to the surgery a few days before treatment runs out. We have a prescription post box at the reception desk to save you queuing. You can also order your repeat prescription on-line (age over 16 only). Please ask the receptionist for a registration form containing a password to access the facility.

Because of the risk of errors, repeat prescription requests cannot be taken by telephone.

The practice policy is that repeat prescriptions will be available to collect from reception no later than 48 hours from the time they are received. If you enclose a stamped addressed envelope for the prescription to be posted out to you, please allow time for postal delays. You will still have to see the Doctor at intervals as he/she requests, in order to check the effect of the treatment. For medicines you require that are not on this slip, please make an appointment to see the Doctor or request a telephone consultation.

When collecting your prescription, we will check that you are the correct person collecting the correct prescription.

ELECTRONIC PRESCRIPTIONS

The practice now has the ability to send your prescription straight to the pharmacy of your choice electronically.  This will mean that you do not need to return to the surgery to collect the prescription, although you will still need to bring it to the surgery or request online.  You will need to nominate a pharmacy in order for this to happen.  This can be done by completing a form at the surgery or at your preferred pharmacy.  If you have nominated a pharmacy, all future repeat prescriptions will automatically be sent there for you to collect.  The 48 hour policy remains the same for electronic prescriptions.  Please allow additional time for your nominated pharmacy to dispense your prescription before collection.

Please find nomination form below to print off and return to either the surgery or nominated pharmacy:

If you have controlled drugs on your repeat prescription, please note that these cannot be sent electronically and will still need to be collected from the surgery.
