Patient survey: Diabetes education for people diagnosed with Type 2 diabetes

Dedicated diabetes education sessions happen all across the country, however attendance is low.  We would like to understand why this is, and what steps we can make to improve the uptake.  Current uptake nationally is approximately 7% for people diagnosed with Type 2 diabetes.   We know diabetes education is one of two diabetes indicators Clinical Commissioning Groups (CCG) are measured on and this is reported in the CCG Improvement and Assessment Framework. 

We are inviting patients to complete a survey about diabetes education to provide invaluable experience and insight into what the incentives and challenges are as well as considering different options for delivery of the education.  

We are inviting people diagnosed with Type 2 diabetes to complete this survey. To complete, please click on link below:

The survey is voluntary and anonymous.  It should take a few minutes to complete and the survey will close on Monday 10th September 2018.

Non NHS Services

Cheveley Park Medical Centre does offer Non-NHS services.  These include:-

Private employment sick note

Letter to school (basic statement of fact)

Letter to third parties (basic statement of fact)

Travel certificate eg stating no reason to limit travel

Claims forms eg holiday cancellation, sickness/accident benefit

Medical reports (patient request, no exam required) eg firearms, scuba diving

Medical examinations (basic) eg DVLA/HGV/PSV/Taxi Driver/basic pre-employment 

Medical examinations (complex) eg detailed pre-employment

Power of Attorney witnessing (where appropriate)

Private consultation with nurse/GP

Private ECG/Spirometry

Private sick notes

Private blood tests

Travel Vaccination Service (non-complex only) - we offer Hepatitis A (although currently not available due to a manufacturing problem) and typhoid.  Any other vaccinations required for travel must be sourced privately and are not available at the surgery.

Please ensure you explain your requirements to the receptionist and have the relevant documents and payment available prior to the request being carried out.
